I came across this issue when adding a custom domail:

We are aware of an issue affecting SSL certificate issuance for custom domains. Users adding or renewing custom domains may see certificates fail to provision.
This is caused by an upstream incident with our certificate provider. We are monitoring the situation and will provide updates.
