The TCP proxy endpoint that's failing: Database Public Endpoint
The error pattern (intermittent ECONNREFUSED)
That internal endpoints work fine
That this started within the last 24 hours with no config changes or dramatic user behaviour changes
The database health metrics are normal
Other health checks are normal within railway, but external Select 1, queries fail intermittently.
Seemed to have started around 11:35pm EST last night.

Hey, have you tried to remove and readd the TCP proxy and see if that changes anything?

It hasn't occurred to us to try. Especially since we have seen this before in the past, but it went away. Multiple times. With no changes on our end, or noticeable traffic differences. I'm more so wondering if there's an issue with the US East region. We signed up for the outage alerts and issues, and we decided to create a case to see if it's just under reported at the moment, due to it's random nature. The connection isn't down out right. Maybe a few times every hour or two

Following up, I noticed the announcement of issue and resolution of the US East issue yesterday.
I just wanted to provide additional information that we are still getting health check failures periodically still. Can we confirm if the issue is resolved?
